Your application must include:

• Curriculum vitae of principal doctor; and partner(s) or assistant(s), if applicable
• Certified true copy of principal doctor’s Annual Practicing Certificate (APC); of partner(s) or assistant(s) as well, if applicable
• Certified true copy of Practitioner’s Full Registration Certificate of partner(s) or assistant(s) as well, if applicable
• Certified true copy of Full Registration Private Healthcare Facilities & Services Act Certificate
• Location map of clinic
• Photographs (view from the outside and a few from the inside) of the clinic
• Facilities available and range of services offered
• Clinic service hours
• Proposed (range of) charges for consultation and treatment for common reasons for GP Clinic Visits
